It depends on your plan as to how much it will cover, and you can generally save more if you stay in the carrier’s provider network.

Humana’s Georgia Plans And Coverage
Humana offers a choice of POS and HMO plans to Georgia residents. The number referenced in each name has to do with the deductible an individual is responsible for when visiting providers within the network. After paying the deductible, the coverage starts to pay for your care. You will find plans under the platinum, gold, silver, and bronze levels. The higher quality metals designate plans that offer more coverage yet require higher premiums.
POS Plans
With Humana in Georgia, there is one POS offering under each of the metallic levels, in addition to a catastrophic plan for basic coverage. The POS plans allow you to go to providers inside and outside the network, while network ones cost less.
The catastrophic offering is the Humana Basic 6600 plan. With this plan, there is no charge to you for inpatient and outpatient services when you stay in the network. The price goes up to a 25 percent co-insurance outside the network. The Humana Bronze 6300 plan also won’t cost you anything for your mental and behavioral health treatment and substance use disorder treatment in the network, whether you choose outpatient services or to enter an inpatient treatment facility. If you want to go somewhere outside the network, your costs will be a 25 percent co-insurance.
The Humana Silver 4250 plan covers inpatient and outpatient treatment for mental health, substance abuse and behavioral health at a 20 percent co-insurance for you. This is in the network, while the cost rises to 40 percent for non-network professionals and facilities. The Humana Gold 2500 plan and the Humana Platinum 1000 plan both provide coverage with a 20 percent co-insurance in the network and 40 percent outside the network. With all of these POS plans, pre-authorization is often needed by the carrier before you can enter an inpatient facility.
HMO Plans
With the HMO plans under Humana, you can only visit a provider within the Humana network to receive coverage. The catastrophic plan is the Humana Basic 6600 plan. It covers substance abuse and mental health services completely. There is also no charge for these services with the Humana Bronze 6300 plan.
The Humana Silver 4600 plan comes with a 20 percent co-insurance for inpatient and outpatient care. At the gold level, you have the Humana Gold 2500 plan. This plan also requires you to pay 20 percent for these services. Finally, the Humana Platinum 1000 plan needs you to take care of a 20 percent co-insurance for your mental health and drug or alcohol dependency costs. Similar to the POS plans, you often need to get inpatient care approved in advance.
