Outside the network, you pay an out-of-network deductible and a 50 percent co-insurance.
The BlueCare Essential PPO plan has a $6,600 in-network deductible for a person. For outpatient care, you get the first two visits covered completely, then you need to pay the deductible, and then your visits after that will cost nothing. For inpatient care, you are responsible for the deductible, and then the rest is covered by the carrier. It is the same for out-of-network care except that you have a different deductible amount.
If you have the BlueCare Vital PPO plan, your individual deductible in the network is $6,600. On this plan, you pay the deductible and then the rest is covered, for both in-network and out-of-network inpatient and outpatient services. With outpatient care, you first get three visits covered completely before you’re responsible for the deductible. Also, out-of-network care has a higher deductible.
