It does not cover providers outside the network, but covers in-network care at a 20 percent co-insurance cost to you. You most likely need pre-authorization for your treatment.
Gold Level Coverage
At the Gold level, you have the PCMH Blue EPO 900 plan, which costs a $60 copay for outpatient visits and a 10 percent co-insurance for inpatient treatment. You usually need to get pre-authorized, and out-of-network care is not part of your plan. Another choice is the Health Savings Blue EPO 2000 plan, which includes a health savings account. On this plan, there is no charge for in-network care, whether you choose inpatient or outpatient services. You usually need to get your care authorized in advance. Providers that are not in the network are not covered.
Platinum Level Coverage
For a Platinum plan, consider the Shared Cost EPO 300 plan. Out-of-network care is not covered. In the network, you pay a $20 copay for each outpatient visit or a 10 percent co-insurance for inpatient services. You need pre-authorization for most care.
Outside the metallic levels, a choice available to you is the Major Events Blue EPO 6850 plan. This plan requires no charge from you for in-network care and does not cover out-of-network care. Like the other plans, it generally requires you to get authorization before your care begins.
